Diphtheria is an acute toxin-mediated superficial infection of the respiratory tract or skin caused by the aerobic gram-positive bacillus Corynebacterium diphtheriae. The epidemiology of infection and clinical manifestations of the disease vary in different parts of the world. Historical accounts of diphtheria epidemics have been described in many parts of the world since antiquity. Developed in the late 19th century, the diphtheria antitoxin played a pivotal role in the history of public health and vaccinology prior to the advent of the (...) diphtheria-tetanus toxoids and acellular pertussis vaccine. One of the most significant demonstrations of the importance of DAT was its use in the 1925 diphtheria epidemic of Nome, Alaska. Coordinated emergency delivery of this life-saving antitoxin by dog-sled relay in the harshest of conditions has left a profound legacy in the annals of vaccinology and public health. Lead dogs Balto and Togo, and the dog-led antitoxin run of 1925 represent a dynamic illustration of the contribution made by non-human species towards mass immunization in the history of vaccinology. This unique example of cooperative interspecies fellowship and collaboration highlights the importance of the human-animal bond in the one-health initiative. (shrink)

Preoperative informed consent is a legal and ethical requirement that ensures patients understand a procedure, its associated risks and benefits, alternative treatment options, and potential complications to make an informed decision about their care. This cross‐sectional study evaluated the informed consent process for major orthopaedic surgeries at a tertiary hospital in Nigeria. A self‐administered questionnaire was used to collect data from 120 adult participants. Results showed that many patients do not read the consent form before signing it, and surgeons do (...) not adequately explain the alternatives to surgery, potential risks, and available anaesthetic options. Higher‐educated patients are more likely to read the consent form. Surgeons performed well in explaining the nature of the condition and prognosis, the procedure's potential benefits, and answering patients' questions. The study emphasises the need to improve the informed consent process to ensure patients understand and can make rational decisions about their healthcare. (shrink)

The association of resilience-related factors with frailty is a recent research topic. Dispositional optimism and context sensitivity are two psychological factors that differently contribute to individual resilience. This study aimed at investigating whether dispositional optimism and context sensitivity might contribute to a multifactorial model of frailty, together with established relevant factors such as cognitive and physical factors. This cross-sectional study involved 141 elderly outpatients aged ≥65 years, who were referred to the Geriatrics and Multidimensional Evaluation Clinic of the University Hospital (...) of Messina. We used the following measures: the Mini-Mental State Examination to screen for global cognitive functioning; 4-m gait speed and handgrip strength to measure physical performance; a 35-item Frailty Index to evaluate patients’ frailty status; the revised Life Orientation Test to gauge dispositional optimism; and the Context Sensitivity Index to measure context sensitivity. We found that LOT-R, CSI, and MMSE were all significantly associated with FI. Gait speed was only marginally associated with FI. The present study showed a novel association of dispositional optimism and context sensitivity with frailty among elderly outpatients. These preliminary findings support a multidimensional approach to frailty in which even peculiar psychological features might provide a significant contribution. (shrink)

Summary Liberals, from Mill to Rawls see personal autonomy as paramount in civil society. They see human dignity to consist essentially in personal autonomy, that is, ?in the ability of each person to determine for himself or herself a view of the good life? (Taylor, C. (1992) p. 27). Multiculturalism and ?The Politics of Recognition? p. 57 (Princeton, Princeton University Press). This emphasis on personal autonomy underlies much of liberal emphasis on freedom of conscience, justice, rights and fairness. Its core (...) thesis is that a just society seeks not to promote any particular view of the good life, but enables its citizens to pursue their own ends consistent with a similar liberty for all others. Yet, when some citizens in America attempted to pursue their own view of the good life, they came up against head?on conflict with the liberal state. These groups saw civic education with its emphasis on individual choice, on reflecting and weighing of alternative viewpoints and on personal autonomy, as threatening to their way of life and to their fundamental religious beliefs, and consequently they challenged the state in the courts. What ensued raised profound educational problems for educating children, parental rights, and the right of the State in a liberal democratic society, which are discussed in this paper. (shrink)

Se examina la interacción entre los interlocutores a los fines de: a) identificar los roles discursivos y estrategias argumentativas puestas en juego por ambos participantes, b) analizar las identidades sociales construidas en la interacción y la eventual -y remisa- demarcación de un éthos griego y bárbaro, c) especular sobre el significado del episodio en el contexto (...) de las Historias y las connotaciones ideológicas de la presunta relación entre la autocracia persa y la realeza espartana. The paper offers a discursive analysis of the dialogue between Xerxes and Demaratus, prior to the Persian attack on the Greeks, as dramatized in Herodotus 7. 101-104. The interaction between the interlocutors is closely explored in order to: a) outline the discursive roles and argumentative strategies deployed by either participant, b) address the social identities constructed throughout the interaction and the prospective, albeit hazy, delineation of a Greek and barbarian éthos, c) speculate on the significance of the episode in the context of the Histories and the ideological implications of the alleged relationship between the Persian autocracy and Spartan kingship. This paper proposes a number of improvements to the text of Basil of Caesarea's Homiliae in hexaemeron 4 and 5. The biblical text poses particular problems for the fourth and the fifth homilies. Therefore, the text form of Genesis from these two homilies is discussed first, and then further individual instances from the fourth and the fifth homilies are examined. The passages are presented in the format of a commentary under the assumption that the reader has the GCS edition (...) at hand. (shrink)

The necessary appearance of Clifford algebras in the quantum description of fermions has prompted us to re-examine the fundamental role played by the quaternion Clifford algebra, C 0,2 . This algebra is essentially the geometric algebra describing the rotational properties of space. Hidden within this algebra are symplectic structures with Heisenberg algebras at their core. This algebra also enables us to define a Poisson algebra of all homogeneous quadratic polynomials on a two-dimensional sub-space, $\mathbb{F}^{a}$ of the Euclidean three-space. This enables (...) us to construct a Poisson Clifford algebra, ℍ F , of a finite dimensional phase space which will carry the dynamics. The quantum dynamics appears as a realisation of ℍ F in terms of a Clifford algebra consisting of Hermitian operators. (shrink)

Recognizing that contemporary attacks upon theology question the intelligibility rather than the truth of theological propositions, the author begins with comments upon the discussion between A. G. N. Flew, R. M. Hare, Basil Mitchell and others in New Essays in Philosophical Theology. After pointing out that contemporary objections to theological discourse are far from conclusive, he suggests that problems arising with respect to such discourse are to be resolved by a return to a theory of knowledge which holds that (...) intellection is intuitive as well as merely discursive. Ever since they were first published, the works of Bernard Mandeville have met with a few careful readers as well as with a larger number of stupid or unscrupulous assailants. Both classes are faithfully recorded at the end of F. B. Kaye's splendid edition of The Fable of the Bees , which has helped to revive interest in Mandeville, and which has moulded the current estimate of his ideas: the treatment of Mandeville in such a work as Basil Willey's (...) Eighteenth Century Background is confessedly based almost entirely on Kaye. My purpose in this paper is to suggest some modifications of the account given by Kaye, and to make claims for Mandeville's importance in one particular field, the relation of ethics to politics. Paternal authority is recommended as a valid Christian resource for conflict resolution in biomedical (and other inner-familial) decision making. Its bases are explored in view of the two-fold creation account in Genesis, interpreted in the light of the Pauline theology. In addition, a theological account is proposed that portrays the taxis between husband and wife as a condition under which humans can seek to emulate the inner-Trinitarian love. The relationship between that love (as portrayed in St. Basil’s On the (...) Holy Spirit), the love between God and man, and the love between the sexes (as further extended to the children in a family that constitutes a “small church”), is described in terms of an interplay between the Divine offer of grace and the human response of obedience. In framing their marital love according to the model of Christ and His Church, couples can also profit from the ascetical therapy predicted for their fallen nature in the “curse” that drove our forefathers from Paradise. This book is a new attempt to clarify what is at issue in the contemporary realism debates and to suggest which form the controversies ought to take. Wright has contributed to the-se debates for quite some time and essentially taken the anti-realist side (witness the papers collected in Realism, Meaning and Truth, 1987, 21993, and the forthcoming Realism, Rules and Objectivity, both Oxford: Basil Blackwell). In Truth and Objectivity however, he takes a step back and sketches a neutral ground (...) upon which both sides could agree in or-der to define their oppositions clearly, thus enabling fruitful discussions. His methodolog-ical suggestion for a realism debate in a given assertoric discourse is that both sides should agree on a “minimal” concept of truth for that discourse and then see whether ascent to a more metaphysically substantial concept of truth is warranted, which would constitute a realism for the discourse in question. If Wright had managed to set the agenda in a way that does justice to both sides, this book would have constituted a major contribution to contemporary epistemology and metaphysics. (shrink)
